GENERAL SUMMARY: The Case Manager has the responsibility for referrals, managed care updates and regulatory/legal requirements for agencies contracted with CCHCS infusion.
Job Requirements
Graduate of an accredited School of Nursing , A.D.N. or B.S.N. program, preferred but not required. Knowledge of payor service guidelines and Joint Commission regulations. Excellent presentation and communication skills required. Detail oriented. Excellent organizational skills. Relationship building skills. 2 years of Home Care experience. Prior insurance case manager or educator experience preferred. Current license as a Registered nurse in the State of Ohio required. Mobility, grasping, bending, reaching, sitting, standing.
