The Hospice RN provides/coordinates/documents nursing carefor Hospice patients. Implements appropriate interventions and facilitates coordination with the Interdisciplinary HospiceTeam.
This is a Fee-For-Service Position
KEY RESPONSIBILITIES:
â—Ź Team Responsibilities: promotes and maintains communication with other disciplines regarding patient/caregiver care and needs. Confers with supervisor and attends team meetings
â—Ź Documentation Responsibilities: Documents identified problems and nursing interventions appropriately on admission and throughout patient stay. Documents Interdisciplinary planning, following agency policies and procedures
â—Ź Professional Responsibilities: Promotes and monitors communication with medical staff regarding patient care and agency function. Maintains the highest level of professionalism and conduct at all times.
â—Ź Clinical Responsibilities: Initiates and performs intervention to meet patient/caregiver needs. Observes progress and alters interventions based on changes in the patient's condition/environment. Provides supervision of aide staff per agency policy. Maintains patient confidentiality. Carries an acceptable caseload effectively according to agency policy
â—Ź Administrative Responsibilities: Turns in paperwork/documentation according to agency policy. Participates in Chart Audits/Reviews and Quality Improvement processes.
â—Ź Accepts assignments outside the normal duties when such fulfills agency need

Job Requirements
Education: Associate Degree in Nursing or higher
Experience: two year experience as an registered nurse
Licensure, Registry or Certification Required: Registered Nursing License in South Carolina
