Job Description
The Hearth at Greenpoint is seeking a Full Time LPN for our 3pm-11pm Shift.
Skills and Knowledge Required:
Current NYS LPN license required.
Basic skill in delivery of patient care, use of equipment and instruments. Good interpersonal communication skills; accurate reading/recording of information.
· Knowledge and skills necessary to provide care appropriate to the age of patients served. Must demonstrate knowledge of the principals of growth and development over life span and possess the ability to assess data/interpret appropriate information needed to identify each patient’s requirements to age specific needs.
· Ability to use age appropriate communication skills.
· Ability to: Follow oral/written instructions; communicate work effectively with a wide variety of people/patients of all ages; function in stressful or crisis situations; accurately read/record data; be flexible to changing work priorities; assist in assessing and responding to patient needs.
· Must be able to perform CPR and other technical skills requiring lifting or turning.
· Must be able to position patients.
· Must be able to wash hands thoroughly and frequently.
· Ability to perform basic duties typically acquired through:
o Completion of accredited training program for Licensed Practical Nurses
o License/Certification required: Current New York State LPN license, or proof of application for license. Current CPR certification required.
Responsibility:
· For maintaining confidentiality of information:
o Works with patient records/information.
Contactsâ€"Internal/External:
· Daily interdisciplinary contacts in performing services to and for patients.
· Daily contacts with patients, families, visitors, physicians, and nurses.
· Interacts with community agencies with direction of the RN.
Special Effort Required:
· Physical stamina, considerable walking, standing, lifting/moving of patients.
· Able to perform duties and effectively respond in stressful environment and situations, mentally and physically.
· Care of effort to avoid injury and infection.
· Must be mentally capable of processing information quickly.
· Demonstrates flexibility in response to changing situations, workload, and/or patient assignments.
· Must be able to lift 20-30 lbs. (Patient and/or equipment, including overhead).
· Must be able to push and/or pull 20-30 lbs. (Patient and/or patient equipment).
· Must be able to respond immediately/move quickly to emergency situations.
· Must be able to use hands, wrists, arms, and both legs simultaneously. (May not work with “devices"â€"splints/casts that limit ability to perform duties or impede hand washing).
· Must be able to stand and walk about 6-8 hours at a time.
· Must be able to squat, stoop, bend, twist, and kneel in order to meet patient needs and/or in order to work with patient equipment.
· Must be able to arrive at work during severe weather conditions (snow, wind, hail, thunderstorms, dust, and extreme heat/cold).
· Must be able to participate in CPR on appropriate population.
· Must be able to remain awake and alert at all times.
Working Environment:
· Exposure to communicable disease, infection.
· Risk of injury from lifting/moving patients and/or equipment, injury from combative patients.
Essential Job Functions:
At an entry or introductory level, responsible for all patient care assigned, and standards of practice with the direction/support of the RN. Participates in health care team approach for patients. Administers prescribed treatments and medication after demonstration of clinical competency and in accordance with nursing policy and procedure. Performs basic nursing activities/activities as noted on procedure index, such as direct patient care, sterile/non-sterile treatments, collecting specimens, monitoring vital signs, and intake/output. Documents completely, accurately and timely. Attends/participates in in-service education quality assurance programs and committees as assigned. Effective communication skills required. Physical effort: lifting/moving patients and/or equipments. Alters activities/behavior to reflect/ensure adequate care appropriate to the age of patients served (i.e. geriatric and adult).
Associated Job Duties:
· Contributes to and participates in various aspect of the nursing process under direction of the RN.
· Performs other job duties as assigned.
