Faith Presbyterian Hospice, a member of the Presbyterian Communities and Services family, is committed to providing exceptional care with a complete range of services, complementary therapies, and special programs. We are currently seeking an LVN Referral Coordinator to add to our team of caring, compassionate individuals.
Schedule: Monday - Friday 8:00 a.m. - 5:00 p.m.
ESSENTIAL DUTIES:
- Communicate Hospice services to prospective patients, families, and referral sources
- Coordinate the scheduling of admission representatives and admission staff to ensure appropriate admission coverage
- Gather initial financial information and work with the Billing Department to confirm benefits
- Supervise the scheduling of patient/family appointments
- Work with clinical managers to ensure smooth admission process
- Coordinate follow up with referral sources and marketing representatives, pertaining to their prospective territories
- Maintain and update documentation and tracking of each referral until conversion or NTUC
- Obtain Doctor’s orders as required
- Assist with the gathering and tracking of admissions statistics
Job Requirements
- Valid Texas LVN required; One to two years previous experience in an admissions or intake department preferred
- Excellent interpersonal, organizational and verbal communication skills, to include a courteous phone voice and helpful demeanor
- Demonstrated proficiency in Microsoft Office applications, as well as hospice-related software
