Job Description
Park Avenue Health Care is a prominent multi-specialty group with a 25-year history of providing excellent medical and behavioral care to residents of Long Term Care facilities.
We currently have a full-time position available in Troy, NY, for a certified Nurse Practitioner to provide primary care services to residents of long-term care facilities. NP is required to perform medical examinations and outline treatment plans for residents. Schedule is daytime hours, Monday through Friday, 40 hrs. per week.

Job Requirements
- Certification from ANCC or AANP
- At least 1 yr. of NP experience
- Experience in a long-term care facility desired
