Providence is calling Case Managers!
The Registered Nurse Case Manager is responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence. Will function independently and/or in conjunction with an interdisciplinary team based on the patient's identified needs. Practice is within the scope defined by the Washington State Nurse Practice Act, based on the nursing process model and concept of community health nursing.
We're seeking an individual who performs all duties in a manner that promotes teamwork and reflects the Mission and Philosophy of our founders, the Sisters of Providence.
Position Specifics.
This position is located at Providence Home Services of King County in Renton, WA. This is a full-time day shift position, and will provide in-home patient care in the Downtown Seattle up to the north end of King County.
Job Requirements
Requirements.
Education: Graduate of an accredited nursing program; BSN preferred
Licensure: Current Washington State Registered Nursing License; Current Washington State Driver's license and have access to daily use of an insured, reliable vehicle; Current CPR certification.
Experience: At least three years full time experience practicing as a registered nurse with one year of recent experience (within last three years) clinical experience in a Medicare certified Homecare or Hospice agency/facility doing intermittent visits and case management is preferred.
