for our local DAPHNE (FAIRHOPE), AL office.

Job Requirements

• RN-Case Manager
The Hospice RN assists in the development and supervision of the Plan of Care for each of their patients; they visit their patients on a regular basis; perform physical assessments; monitor medications; work closely with the physician & other team members to manage pain and other symptoms; educate & provide compassionate support; help make every day count.  Many nurses who have worked in other settings say that hospice allows them to spend quality time with their patients and families, develop caring relationships and enables them to utilize a great deal more of their professional training.  Must have reliable transportation.

• RN Admission Coordinator
This person is a compassionate and energetic nurse that is both an integral part of the community relations team and finds great reward in taking the time to meet with families in becoming the first face of hospice for them. This Registered Nurse will integrate the needs of patient’s families, perform patient assessments, as well as collect data through histories and physicals, hospital discharge plans and physician notes to assess the appropriateness of a referral and develop an Initial Plan of Care for the patient.  This individual must communicate effectively with the physicians to attain orders, educate families on palliative care and organizational abilities to coordinate with the Care Team.  Excellent documentation and time management skills are required. Flexibility in meeting the scheduling needs of patients and families are essential. This person will report to the Community Relations Director.
