Job Description
This position is responsible for providing social services counseling services to home health patients and caregivers according to department policy, procedures, and department guidelines. 1. Conduct initial psychosocial assessment within the appropriate time frame after. 2. the patient s admission. 3. Coordinate and provide the delivery of social services in accordance with the interdisciplinary plan of care. 4. Continue assessment of patient/family psychosocial needs in accordance with the interdisciplinary plan of care to assist the patient/family in dealing with the psychosocial aspects of illness as evidenced by documentation. 5. Participate as an interdisciplinary team member. 6. Participate in case conference at least every two weeks. 7. Utilize community agencies and alte rnative resources to assist patients and their families in resolving unmet health care needs, financial and environmental difficulties. 8. Participate in orientation, inservice and staff meetings as scheduled. 9. Provide required documentation within established time frames. 10. Report pertinent observations to attending physician, Case Manager or Patient Care Coordinator. Preferred master s degree (M.A.) from a university or college accredited or approved by the Council on Social Work Education; or Bachelor's degree (B. A.) from four year college or university accredited or approved by the Council on Social Work Education working under the supervision of a Master s prepared Social Worker, and one related experience in a health care setting. Must maintain a current Texas State Driver 's License and proof of liability insurance on file. Must hold a current Social Work license in the state of Texas.
