Job Description
To resolve social or emotional problems that are, or are expected to be, an impediment to the effective treatment of the patient’s medical condition or their rate of recovery.
Collaborates with professional staff on psychosocial problems and appropriate interventions.
Counsels clients & families regarding community resource availability.
Develops and implements client plan of care.
Monitors quality and appropriateness of services provided.
Acts as resource to professional staff.
Job Requirements
Masters Degree in Social Work. Valid NJ LSW. One year supervised clinical experience. Valid NJ drivers license.
Community health experience preferred. Bilingual preferred.
