Job Description
Position is a member of the Diabetes Care Team, Diabetes Self Management training program. Will be responsible for performing patient assessments and developing education plans and educational goals for patients. Position also participates with other RD educators in classroom education. Position is responsible for program development, patient data collection and physician and other provider interaction and support. Other duties as assigned.
Current Registered Dietician licensure required. Patient education experience required. Certified Diabetic Education certification required.
